Why does shoulder pain happen?
The shoulder is the most mobile joint in the body, which makes it vulnerable to a range of different problems. Shoulder impingement, rotator cuff tears, frozen shoulder and calcific tendinopathy all present with similar symptoms — pain with movement, aching at rest, disrupted sleep — but each requires a different treatment approach.
Shoulder impingement and rotator cuff problems are typically driven by a combination of muscle imbalance, posture and overuse. The tendons become irritated when the shoulder mechanics aren’t working efficiently. Hands-on treatment combined with targeted strengthening almost always produces good results.
Frozen shoulder is a different beast entirely. It develops in three stages — a painful inflammatory phase, a stiffening phase, and a thawing phase — and the treatment needs to be matched to the stage. Early intervention is particularly important: the earlier frozen shoulder is treated, the shorter and less severe the stiffening phase tends to be.
Common contributing factors
- Muscle imbalance or weakness around the rotator cuff and shoulder blade
- Repetitive overhead activity or overuse
- Poor posture placing sustained load on the shoulder structures
- Inflammatory response triggering the frozen shoulder cascade
- Calcium deposits within the rotator cuff tendons (calcific tendinopathy)
