What is Orthotics?

Orthotics are custom-made or off-the-shelf insoles worn inside footwear to correct the way your foot contacts the ground and distributes load through your lower limb. At SHPW, orthotics are always preceded by a full biomechanical assessment — we look at how you stand, walk and move before recommending any device.

The prescription of orthotics is not simply a matter of arch support. The goal is to modify the mechanical forces travelling through the foot, ankle, knee, hip and lower back. By changing how load is distributed, orthotics can reduce pain in the foot itself and also in structures further up the chain — including the knee, hip and lumbar spine — that are being overloaded due to poor foot mechanics.

Not everyone who presents with foot pain needs orthotics. Our assessment identifies whether a structural or functional abnormality is genuinely contributing to your symptoms, or whether exercise rehabilitation, footwear advice and manual therapy will achieve the same result without a device.

Clinical evidence

What does the evidence say?

The evidence for orthotics is strongest in specific conditions where altered foot mechanics are genuinely contributing to the problem. For plantar fasciitis, foot orthoses are a recommended first-line treatment alongside calf and plantar fascia loading. For patellofemoral pain (runner’s knee), there is good evidence that foot orthoses reduce pain when combined with physiotherapy. For lower limb overuse injuries in runners, biomechanically appropriate orthotics can reduce injury recurrence.

The key is appropriate prescription. Generic off-the-shelf insoles have limited evidence for most conditions — the benefit comes from a device that addresses your specific mechanics. That requires assessment first.
