Why does hand & wrist pain happen?

Carpal tunnel syndrome is the most common hand condition we see, and it’s frequently misunderstood. It occurs when the median nerve is compressed as it passes through the carpal tunnel at the wrist, causing the characteristic tingling, numbness and weakness in the thumb and first two fingers. The compression is often made worse by positions that flex or extend the wrist under sustained load — driving, sleeping with a bent wrist, keyboard work.

Many patients are advised that surgery is the only effective treatment. In reality, a significant proportion of carpal tunnel cases — particularly mild to moderate presentations — respond well to conservative physiotherapy management, nerve mobilisation and activity modification. It’s also worth noting that symptoms similar to carpal tunnel can come from nerve root compression in the neck (cervical radiculopathy) rather than from the wrist itself. Distinguishing between the two is an important part of our assessment, as the treatment is very different.
